Coverage of a family pensioner under WBHS 2008

I would request the fellow members to please guide whether the widow of an employee of West Bengal Government whose husband had died during the service period is eligible to enroll herself under WBHS 2008 or not? The widow is getting the family pension from the government and and lives in U.P? What procedure should be adopted for her coverage under WBHS 2008? Kindly guide me to guide her with the reference of the relevant G.O. thanks

Family Pensioner are also eligible to opt for WBHS Scheme as per G.O. No. 3475-F dated 11.05.09 (Attached).
Enrollment Precudure:
1. Family pensioner shall exercise option in Form – I in duplicate along with the undertaking that upon enrolment under the scheme she will forgo the regular medical relief drawn along with monthly pension.
2. Then it is to be submitted to the Pension Sanctioning Authority (PSA). After scrutinizing the option, PSA shall issue a certificate of enrolmement in Form – II in favour of the family pensioner effective from the 1st day of the next month, the certificate is issued.
3. PSA shall send one copy of the certificate to the concerned Pay & Accounts Office / Treasury with a direction to discontinue the drawal of medical relief w.e.f. from the date mentioned.
