I have WBHS card issued by my Kolkata HQ office when I was posted there. Now I am posted in Block office. I want to include my new born baby of 4 months age in WBHS. Kindly let me know the procedure.

New Cards need to be issued for all existing family members as all cards have existing identification No. along with list of family members or simply make necessary correction in the existing cards.

10. Treatment of new-born baby is chargable separately in addition to the
delivery charges for mother. A baby is entitled to the treatment facilities under the
Health Scheme for six months on the basis of Identity Card of parents within which a
separate card is to be obtained for the baby.

4. Treatment charges for new-born baby are separately reimbursable in addition to
delivery charges of the mother. Baby is entitled to get treatment on the basis of identity card
of parents up to six months of age. A separate identity card under the Health Scheme should
be obtained within that period.

The case has been forwaded to my Directorate HQ but no reply has so far been received. If any one has come to know the similar facts than please share.
