Happy new year to every body
One of my staff lost Health Card (W.B.H.S.2008) which was issued and accordingly he applied for new card, and when his prayer with note forwarded to DDO, then DDO asked to Quote Rule in this regard,
So my submission is, if any body has any idea how to get new card in this regard, it would of immense help.

So far I can recall, there is nothing specific in this regard in the Rules. Since it’s basically a identity card, the procedure of issuing new one may be in accordance with issuing a new I.card (the red card issued for govt employee). Heath identity card consists of two parts of which the issuing authority retains the first part and the second part is handed over to the employee concerned. Data are available from the second part and on the basis of the application (supported by a copy of G.D. in Police Station) a new card marked as ‘Duplicate’ may be issued.
If there is nothing written in the rule about issuing a new card, it also implies that it has not even been restricted by the rule.
